MERRILL FAY ARENA – HOCKEY WAIVERParticipation Agreement Release Of Liability Waiver Of Claims And Assumption Of Risk Release and Waiver of Liability, Assumption of Risk and Indemnity Agreement
The player acknowledges and agrees to the following conditions of participation:
1.            I understand that ice hockey is an exciting and dangerous sport involving risks both known and unknown.
2.            I agree to assume all risks of participating in any event associated with the Merrill Fay Arena Adult Men’s Ice Hockey and further agree to hold harmless the Merrill Fay Arena Adult Ice Hockey and designated Volunteer Organizers/Facilitators of Merrill Fay Arena Adult Men’s Ice Hockey and for any incident and/or injury incurred while participating Merrill Fay Arena Adult Men’s Ice Hockey and, regardless of the cause.
3.            I further agree to release and discharge the above parties from all liability claims, demands, losses or damages caused by or alleged to be caused in whole or part by the negligence or alleged negligence of the above parties.
4.            I agree to exercise good sportsmanship during all ice hockey activities and to be supportive of coaches, officials and volunteers. I understand that all play is recreational and non-checking. Sportsmanship is expected and rough play is not permitted. I understand that my disorderly conduct or conduct dangerous to myself and others, as determined by the Merrill Fay Arena Adult Men’s Ice Hockey and or designated representatives, including designated Volunteer Organizers/Facilitators, may result in suspension or revocation of my participation.
5.            I understand that payment in advance of play is required (no pay =no play).
I HAVE READ THE ABOVE AND AGREE TO THE TERMS AND CONDITIONS
Binding Effect Of Agreement: In the event of my death or incapacity, this agreement shall be effective and binding upon my heirs, next of kin, executors, administrators, assigns and representatives.          
Entire Agreement: In entering into this Agreement, I am not relying upon any oral or written representations other than what is set forth in the Agreement.
I HAVE READ AND UNDERSTAND THIS AGREEMENT AND I AM AWARE THAT BY SIGNING THIS AGREEMENT I AM WAIVING CERTAIN LEGAL RIGHTS INCLUDING THE RIGHT TO SUE THE ARENA AND THE LEAGUE.
Concussion Waiver:
The risk of injury from activities involved in playing hockey is significant, including the potential for permanent paralysis and death.  While particular rules, equipment and personal discipline may reduce this risk; the risk of serious injury does exist, and, for myself, and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE, INDEMNIFY, AND HOLD HARMLESS the Merrill Fay Arena, their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors and advertisers; from any claims arising out of my participation in these activities.
